ASSESS
We will schedule a consultation to assess, evaluate and discuss the overall needs of the individual and the family.
Our assessments cover an individuals medical history, support system, daily functioning ability, cognitive (memory) and current resources.
P.O.C.
(Plan of Care)
Once we've assessed you overall needs we will develop an individualized Plan of Care that will address you short and long term health, social and environmental needs
MONITOR
Once your loved ones P.O.C. has been developed, a Case Manager will manage and monitor his or her P.O.C. Monitoring consist of but not limited to overseeing of all services put in place, doctor & therapy appointments, working with doctors and specialist, medication management, home management, and so much more.

As we monitor your loved ones services and progress we will advocate and work with all disciplines to ensure that your loved ones needs are being met and have the BEST quality of life. We will stay in constant communication with you regarding any needs, their progress, observations and recommendations.
